We need the name of each person who registers for Mentone, so fill out this form for each person in your party and add to your cart (i.e one item per person in your party at checkout).

Before you checkout, review your cart for errors. If you see an item in error, delete it and add the item again. Repeat until you've corrected all errors, then checkout.

If later you decide to cancel, we offer a full refund minus processing fees ($5 per adult, $3 per child aged 8-14).
